Key Notes on the Treatment of Hyperhidrosis & Sweating

Hyperhidrosis treatment refers to the methods used to control excessive sweating. In essence, it is a condition in which the body produces more sweat than usual, which can impact the patient’s quality of life.

Hyperhidrosis:

  • Involves excessive sweating, especially in areas like the palms, face, soles, armpits.
  • It can be genetic (primary hyperhidrosis) or caused by another medical condition.
  • Sweating may occur even without heat or exercise.

What Areas Can Be Treated?

underarm sweat

Underarms (axillary hyperhidrosis)

In order to get rid of armpit sweat, hyperhidrosis injections can be the best solution.

sweating hand

Palms (palmar hyperhidrosis)

Palmar hyperhidrosis botox can be injected intradermally in each hand.

feet sweat

Soles of the feet (plantar hyperhidrosis)

It works by intradermal injection into the plantar skin, where it blocks acetylcholine released at sweat gland nerve endings.

head sweat

Scalp & Forehead

Manageable with onabotulinumtoxinA injections, which cholinergic stimulation of eccrine glands, reducing sweating for up to 6 months.

What is the relationship between sweating and the sympathetic nervous system?

For most people, sweating is a natural result of exercise or hot weather. However, for those who suffer from hyperhidrosis (excessive sweating), it is much more than that. Before starting any treatment, it is necessary to identify the underlying cause of your sweating.

The Sympathetic Nervous System and False Alarms

Under normal conditions, the sympathetic nervous system activates the sweat glands when body temperature rises. However, in patients with hyperhidrosis, the brain continually sends signals to the sweat glands, even in the absence of physical triggers such as heat, intense exercise or stress. Botox treatment works by physically blocking these signals from reaching the sweat glands.

Primary Hyperhidrosis vs. Secondary Hyperhidrosis

There are different types of excessive sweating:

1.Primary Hyperhidrosis

It usually begins in childhood and adolescence. Sweating is intense in the palms, soles, armpits, or one area of the face. There is no underlying medical condition; the problem is directly related to overstimulation of the nervous system.

2. Secondary Hyperhidrosis

If the sweating started suddenly later in life, or if it occurs as “night sweats” affecting the whole body, it may be caused by:

  • Hormonal changes (e.g., Menopause)

  • Metabolic disorders (e.g., Diabetes, low blood sugar)

  • Thyroid issues (e.g., Hyperthyroidism)

  • Neurological conditions (e.g., Parkinson’s disease)

How Botox Works for Excessive Sweating?

Your doctor will inject Botox – Botulinum Toxin Type A into the excessive sweating area. Therefore, it blocks the nerves that activate sweat glands.

Professionals use it for underarms, but it is also beneficial on palms and soles, as well. You can see the results in 3 to 7 days and last up to 6 months.

Who is Hyperhidrosis (Sweating) Treatment for?

Before determining the patient profile for treatment, clinicians need to learn the severity of daily sweating. Your medical past, clinical assessment and tools like the Hyperhidrosis Disease Severity Scale can be useful.

How are the Botox injections given?

First, your doctor will use an iodine-starch test to identify the areas. Then, they will make several small injections into your skin with very fine needles. The procedure usually takes about 20 minutes or so.

Before the underarm sweat treatment, the cosmetic doctor marks the young female patient's armpit.

Is It Safe?

Yes, Botox is a clinically proven and safe product when used by professionals. Turkey’s top dermatology clinics follow the international hygiene and safety codes. The International Hyperhidrosis Society says that people who get Botox injections have found that their excessive sweating goes down by more than 85%. Botox is used to treat various types of hyperhidrosis, but at the moment, it’s only FDA approved for excessive armpit sweating.

Risks and Side Effects of Botox Treatment for Hyperhidrosis

Botox works by temporarily paralyzing the nerve signals that stimulate sweat glands. The situations that may arise during this process should be examined under 3 major categories:

Common and Temporary Side Effects (Specific to the Application Site)

  • Injection site reactions may occur. For example, redness, swelling, and tenderness.
  • Micro-bruising is common. It can occur, especially in areas where the skin is very thin, such as the armpits and palms, as a result of hitting capillaries.
  • Expect mild pain. Injections in the palms and soles of the feet, in particular, may cause tingling during and after the procedure because these areas contain many nerve endings.

Regional and Functional Risks (Muscle Weakness)

  • Weakness in hand functions may occur. In palm applications, if Botox accidentally reaches the small muscles at the base of the thumb or between the fingers, temporary weakness in fine motor skills (such as buttoning or writing) may occur.
  • There may be effects on the eyelids and forehead. In facial or scalp applications, temporary asymmetries such as drooping eyebrows or eyelid drooping (ptosis) may occur as a result of the toxin spreading.

Rare Occurrences and Systemic Risks

  • Compensatory sweating. Although not as common as surgery (ETS), when sweating stops in one area of the body, it has rarely been observed that sweating increases slightly in another area (e.g., back or chest).
  • Flu-like symptoms. Some patients may experience mild fever, fatigue, or headache within the first 24-48 hours after the procedure.
  • Botulism-like effects. In very rare cases, conditions such as difficulty swallowing or shortness of breath may require urgent medical attention (however, this risk is extremely low at cosmetic doses).

Who is at Risk? (Contraindications)

  • Individuals with neuromuscular disorders such as ALS, Myasthenia Gravis, or Eaton-Lambert syndrome.
  • Individuals who are pregnant or breastfeeding. Due to insufficient clinical data, it is not recommended for safety reasons.
  • Certain antibiotics (such as aminoglycosides). These may increase the risk of muscle weakness by enhancing the effect of Botox.

The main factor in minimizing risks in excessive sweating treatment is the depth of the toxin. Botox should be injected intradermally (into the skin) for sweating treatment. If it is wrongly injected deeper into the muscle tissue, the risk of muscle weakness mentioned above increases significantly. Therefore, the procedure must be performed by a dermatologist or plastic surgeon who is proficient in anatomy.

Frequently asked questions

Underwarm sweating botox is usually permanent for a minimum of 6 months and a maximum of 8 months. Patients start to see the results of the treatment in an average of 5 days, after a week at the latest.

No it is not, it is an FDA approved safe treatment. Botulinum toxin therapy is great for reducing sweaty underarms by stopping the sweat glands in the skin from working. It also keeps the skin looking normal and doesn’t make you look sweaty. The procedure is usually done in a clinic setting under aseptic conditions. You won’t need any downtime, so you can get on with your day straight after.

ETS surgery is a permanent in most cases; stops palmar sweating byy severing sympathetic nerves. Despite that, 50 to 80% of patients develop compensatory sweating elsewhere. If you’re looking for semi-permanent options, you can give a chance to iontophoresis. But if you would like to try temporary solutions, botox, antiperspirants, and oral anticholinergics can control sweating, too.

Surgeons have not yet found a permanent solution for focal hyperhidrosis. However, we can largely control generalized hyperhidrosis with surgical or non-surgical sweating treatments.
