SMP (scalp micropigmentation) as a formalized procedure gained traction in the early 2010s, though practitioners were experimenting with scalp tattooing long before that. The SMP technique spread through the UK and US over the following decade, then globally through social media. It has been a common cosmetic procedure in Turkey for about 10 years.

Scalp micropigmentation is a non-surgical cosmetic procedure which was developed to combat hair shedding. In the micro-pigmentation, a practitioner deposits tiny dots of medical-grade pigment into the upper dermis of the scalp. The aim is replicating the appearance of hair follicles. When scalp micro-pigmentation is done well, the result is the visual impression of a closely shaved head, the look of increased hair density in thinning areas, and a more defined hairline appearance.
SMP procedure does not grow hair but it creates the look of hair. The procedure goes by several names. You may also see it called trichopigmentation, dermopigmentation, hair follicle simulation, hair optical simulation, or simply a hair tattoo. These all refer to the same general technique, though there are variations in depth, pigment type, and equipment between practitioners. Unlike a conventional body tattoo, SMPs target only the papillary dermis (the upper layer of skin) roughly 0.3 to 1.5 mm deep. This shallower placement is intentional. It produces finer dots and avoids the ink migration that causes deep tattoos to blur and spread over time. The tradeoff is that SMP fades faster than a tattoo. Most practitioners frame this as a feature rather than a flaw, since it allows for color correction and natural adjustment as the client ages.
The pointillism in SMP is borrowed from fine art. Just as impressionist painters built up images from thousands of individual dots of color, SMP practitioners place tiny dots of pigment across the scalp to create the illusion of hair follicle density. Each dot is roughly 1 to 2 mm in diameter. Just wide enough to suggest the cross-section of a shaved hair shaft.
A practitioner works in passes. The first session establishes a base layer at relatively low density. It’s usually around 30 to 40 percent of the target. The second session builds on that, and the third refines edges, adjusts tone, and fills gaps. Working in layers allows the pigment to settle and the skin to heal between appointments. With this way, you can see more natural results than trying to achieve full density in one sitting.
The stippling pattern matters as much as the dot size. Placing dots in a completely uniform grid can look artificial. That’s why experienced practitioners randomize spacing, vary dot size slightly, and pay attention to the direction of natural follicle growth in each area of the scalp. Here is the goal creating a shadow but not a connect-the-dots outline.
Although both terms are often used interchangeably, there are technical differences between them. Essentially, both involve injecting pigment under the skin using a needle. However, the key differences lie in the details.
Both are very different methods used to hide baldness. While hair transplantation is a medical surgery, micropigmentation of the scalp is a cosmetic procedure.
The combination of hair transplant and SMP combination is increasingly common. A patient gets a transplant to restore volume where possible, then uses SMP to camouflage the FUE extraction sites in the donor area, fill in lower-density zones, and sharpen the hairline. The two procedures are complementary, not mutually exclusive.
SMP is typically performed on the scalp, but it is also used on the eyebrows. Microblading, on the other hand, is a technique performed specifically on the eyebrows to make them appear fuller for people with sparse eyebrows. Although it may not produce results as realistic as SMP, microblading can solve your sparse eyebrow problem for up to 1.5 years. SMP provides longer-lasting results and a more natural look, but trying microblading first might be a practical option.
Microblading creates hairlike lines in the brows using a handheld blade tool that drags pigment through shallow cuts in the skin. It is designed to produce the impression of brow hairs.
SMP produces dots, not strokes. The technique is fundamentally different because scalp hair looks different from brow hair. A shaved scalp shows circular follicle openings, not visible hair shafts. Trying to replicate that with microblading strokes produces results that look like drawn-on hairs, not stubble.

SMP began gaining popularity worldwide in the early 2010s, though practitioners had been experimenting with scalp tattooing techniques long before then. In Turkey, hair transplants were the primary solution for hair loss until 2012. While they are still the most common choice, the arrival of SMP in 2012 introduced aesthetic solutions for those looking for quick cosmetic options. Since the 2016–2018 period, SMP has become steadily more popular.
Today, Istanbul features a wide range of SMP providers, from training academies to luxury cosmetic clinics. Vantage Clinic is one of the best SMP clinics in Istanbul. We have been performing scalp simulation procedures for many years, supported by experienced Turkish scalp micropigmentation specialists and a strong medical team.
Most people with hair loss are candidates. The factors that make someone a good candidate:
If you have stable hair loss…
If your hairline is still receding or your thinning is still progressing, adding pigment now may create inconsistencies later when surrounding hair continues to thin. A six-month period of stable loss is a reasonable benchmark before proceeding.
If you have realistic expectations…
SMP creates the impression of hair. It does not create hair. Patients who want a shaved-head look or who want to reduce scalp visibility tend to be satisfied. Those expecting the results of a hair transplant from a pigmentation procedure rarely are.
If you have a healthy scalp…
Active skin conditions affecting the scalp, such as psoriasis flare-ups and eczema, can interfere with pigment retention and healing. These conditions must be treated before beginning the hair simulation procedure.
If you have darker skin undertones relative to the pigment…
The higher the contrast between the scalp skin and the simulated follicle dots, the more dramatic the SMP result appears. This is not a disqualifier for lighter-skinned patients, but it does affect the visual impact.

Women can also undergo a natural SMP treatment that matches their hair color to fill in areas of thinning hair. This treatment can conceal hair loss for many years. However, if your hair loss continues, it may become apparent that you’ve had this hair tattooing procedure done. Therefore, it’s advisable to first undergo a treatment to stop your hair loss and then have SMP done
Nowadays, because the technology behind this hair-like tattooing has advanced so much, it has become really difficult to tell the difference. However, at some beauty salons, even non-professionals are able to perform this procedure. SMP will look artificial, especially if it’s done with a pigment that’s too dark and doesn’t match your hair color. However, SMP that’s tailored to your hair and skin tones will definitely not look artificial.
Similar to hair transplants, SMP involves small, pinpoint-sized incisions, and these incisions may cause itching as they heal. For this reason, you can relieve the itching using foams and mesotherapy serums like Perseus Hair Care Foam or Meso Set specifically formulated for hair transplants.
Side effects include itching, mild swelling that lasts a few days, and mild redness that lasts a few days. In rare cases, if SMP is not performed under hygienic conditions, side effects such as infection may occur. Please have the procedure performed by medical specialists at a professional clinic. Be cautious when choosing beauty salons, tattoo artists, or hair salons, even if you think they might be more affordable.
