Nipple-areola complex reconstruction

What is areola surgery?

Specialists perform areola-nipple complex reconstruction to recreate the nipple and surrounding areolar tissue for patients who have undergone mastectomy, lumpectomy, or other breast surgeries. Commonly, doctors combine it with implant-based breast reconstruction, autologous tissue (flap) reconstruction or oncoplastic techniques to restore areola, breast, and the dark skin around your nipples.

Patients often seek nipple surgery to boost self-confidence and enhance body image following a breast surgery. The procedure restores the visual aesthetics of the breast and helps patients to feel whole and comfortable with their bodies again.

Why do people choose nipple surgery among so many other breast surgeries?

People may have many concerns about their nipples. For example, postpartum nipple changes, injury-related problems, congenital nipple abnormalities, etc. So, ideal candidates for post-mastectomy nipple reconstruction include:

  • Women who have completed breast reconstruction
  • Individuals with trauma-related nipple loss
  • Patients seeking aesthetic restoration of the nipple-areola complex
  • Correcting breast asymmetry.
  • Fixing shape irregularities between the two nipples.
  • Changing the color of the nipple.
  • Reducing the areola size
  • Restoring the natural appearance of nipples removed after breast cancer.
  • Repairing nipples damaged due to accidents, burns, or cuts.
  • Treating congenitally missing nipples, as well as correcting shape deformities and color loss caused by previous breast surgeries.

What are the types of nipple reconstruction procedures?

Several approaches to nipple reconstruction are possible, depending on the anatomy of the patient and the desired outcome.

A skin graft is used to recreate the nipple. This technique is ideal for patients with enough donor skin, as it allows for a realistic nipple projection and feel.

This method uses local tissue from the reconstructed breast to create a nipple. This method is durable and, in some cases, maintains sensation.

Nipple tattoo reconstruction (including 3D nipple tattooing) uses tattoo ink to enhance the appearance of the nipple-areola complex.

How do surgeons decide on surgery?

Turkish surgeons carefully evaluate patients to make sure skin quality, blood supply, and tissue condition are suitable for a cosmetic nipple procedure.

First, they check whether the skin around the breast is elastic and thick enough to support a well-shaped nipple. They also evaluate whether there is sufficient blood circulation in the breast tissue to allow proper wound healing and avoid tissue necrosis. At the same time, the surgeons will carefully review any scars resulting from a mastectomy, lumpectomy or previous operations, in order to determine the most fitting surgical approach. Then, they plan the nipple’s position, size, and shape to complement the reconstructed breast.

Overall, surgeons study the anatomy, vascular system, and tissue condition, and then tailor the surgical plan to achieve a safe and aesthetically pleasing nipples.

How long does nipple reconstruction take?

Nipple surgeries are generally practical procedures that take no more than 1-2 hours. Local flap procedures take between 30 and 60 minutes. Nipple sharing procedures take 45-90 minutes, and tattoo-assisted or 3D nipple illusion procedures take only 20 minutes since there is no intervention on the actual tissue. If doctors are going to intervene on both breasts, the duration may double; additionally, if there is breast reconstruction or implants, the duration may reach 1-2 hours.

Are there risks with nipple-areola reconstruction?

In the local flap method, common risks such as infection, delayed wound healing, minor skin necrosis, and slight asymmetry may occur, as well as temporary changes such as loss of sensation. The risk of infection ranges from 1% to 3% and the most common risk is temporary loss of sensation. In cases where tissue is transferred from the other nipple, asymmetry may occur in both breasts, infection may occur, and the transferred tissue may not receive sufficient nutrients and oxygen. In tattoo-assisted illusions, fading of the tattoo and pigment may occur.

FAQs

Although immediate reconstruction is sometimes possible, many surgeons recommend waiting until the breast reconstruction has healed.

Scarring is a naturally occurring step in the healing process. Surgeons make incisions in inconspicuous places to minimise visible scarring, and scars tend to fade over time.

You won’t feel pain because you’ll be under anesthesia, and antibiotics will suppress any pain for 10 days after the surgery. After that, you can completely control the pain with painkillers until you recover.
